Below is a list of information one should know & exchange with the other driver, after an accident. We suggest you print this accident information form and keep it in your glove box, along with a pen.
Be sure to get...
- Driver's name
- Address
- Phone number
- Driver's license number and state of issuance
- Name of insurance company
- Insurance policy number
- Vehicle make, model, and year
- Vehicle identification number
- Registration number and state of issuance
- Vehicle owner's name, address, and number if different from the driver
- If possible, take photos of the entire scene, damaged vehicles and property
- Obtain witness names, addresses and phone numbers
- File an official accident report with the police
- Notify your insurance company immediately
